It's incredibly rare that this would happen, but the fact is is that it can. When Val and another character do their supers (val doing her scalpel super) at the same time, sometimes the scalpels seem to forget that they are still in a super freeze and travel through it. This leaves Val with absolutely nothing to cover her.

It's not that rare, it happened to me on a regular basis tonight. Not mirror matches, if that matters.
 
Wow that's pretty unlucky, it took me a while to replicate it. I don't think the mirror match thing matters at all, just two super flashes at the same time.
