I suggest you to change the character on the punishing tutorial from Fortune to any other one (Lesson 2, Chapter 2).
A friend had trouble with head toss when trying to punish. As he tried to complete the tutorial, every successful punish was discarded by the head being hit and he wasn't understanding what was happening. He streamed it to me and I managed to replicate it.
